Purpose Engagement of healthcare professionals with patients from diverse cultural and religious backgrounds is crucial in our multicultural society, where miscommunication and errors in medical history taking can lead to incorrect treatment. In particular, Muslim patients may present unique considerations due to their specific cultural and religious beliefs, which can significantly impact treatment outcomes. This study focuses on perioperative medication therapy for patients undergoing upper and lower gastrointestinal tract and pancreatic tumor surgery, specifically examining whether Islamic beliefs were duly considered (...) in medication selection compared to a matching patient cohort. Materials and methods Data from January 2004 to July 2023 were analyzed. Muslim patients were identified using the onomastic method and matched with non-Muslim patients at a 1:3 ratio based on age, gender, and procedure. Analysis included examination of subcutaneous, oral, and intravenous medications, with attention to ingredients and compatibility with Islamic principles. Results Among 5272 patients, only 5 met the study’s inclusion criteria as Muslim patients, undergoing procedures such as anterior rectum resection, gastrectomy, and pancreatic head resection. Their religious affiliations were not documented in the admission records. According to the matched-pair analysis, consistent treatment was performed regardless of religious beliefs. All patients received subcutaneous medication, primarily enoxaparin, instead of fondaparinux, an Islam-compliant alternative. Intravenous heparin was used once for short period. Contrary to Islamic dietary restrictions and the availability of alternatives, capsules containing animal-derived gelatin and other non-compliant medications were administered orally. Conclusion This study underscores the importance of acknowledging Muslim patients’ cultural and religious backgrounds in the perioperative setting, as failure to do so may lead healthcare professionals to overlook their potential alternative medication needs, which are essential for providing tailored medical care in modern societies. Integration of diversity-related topics into medical curricula is essential for better preparing physicians for clinical practice and ensuring patient-centered care. (shrink)

In this paper, we explore a rationalistic orientation in Western society. We suggest that this orientation is one of the predominant ways in which Western society tends to frame, understand and deal with a majority of problems and questions – namely in terms of mathematical analysis, calculation and quantification, relying on logic, numbers, and statistics. Our main goal in this paper is to uncover the affective structure of this rationalistic orientation. In doing so, we illustrate how this orientation structures the (...) way not only individuals but society as a whole frames and solves problems. We firstly point towards some exemplary instances of the rationalistic orientation, specifically regarding science, society, and lifeworld practice. Crucially, we argue that the rationalistic orientation is not merely based on a set of beliefs we could easily correct; but rather, that it is an affective condition tacitly shaping our engagement with the world in an encompassing way. Relating to the work of Martin Heidegger, we argue that what we have called an orientation in the beginning is in fact a rationalistic attunement. This attunement fundamentally shapes the pre-reflective level of how individuals approach the world. We elaborate this claim by showing how the rationalistic attunement concretely manifests in tangible socio-material affect dynamics. In the end, we motivate a critical stance towards this attunement, providing the ability to reflect upon and question instances where this way of framing and solving problems is counterproductive. Fordham von ReynFatuma's* doctors were completely perplexed. It was 2003 and she had returned to the DARDAR clinic in her hometown of Dar es Salaam, Tanzania three times that week with vague complaints of various pains and aches. Her doctors were considering whether these symptoms were (...) due to the initiation of her recent treatment with antiretroviral medications for her HIV infection or related to the last administration of the investigational tuberculosis (TB) vaccine that the DARDAR Health Study was evaluating. Fatuma's doctor, Dr. Lillian Mtei, consulted with Dartmouth's chief of Infectious Disease, Dr. Ford von Reyn, who happened to be in Dar that week on a site visit. Neither physician could determine the basis for her symptoms. Dr. Mtei returned to the exam room once more and this time emerged with the answer: Fatuma had just learned that her three year old daughter was HIV-infected, likely contracted from Fatuma during or shortly after she gave birth to her daughter. Fatuma believed that her HIV diagnosis was a death sentence for her daughter since antiretroviral therapy was not widely available for children.Dr. von Reyn, Dartmouth's lead clinical researcher in Africa and a father, knew too well the harsh reality of inequities in a resource-limited country. He turned to Dr. Mtei and said decidedly, "we need to do something about this."Thus the DARDAR Pediatric Program was born.Pediatric HIV: A Neglected DiseaseThe need for HIV treatment among children in Tanzania, as in most of sub-Saharan Africa, is compelling. At the end of 2009, approximately 160,000 children aged zero to fourteen in Tanzania were living with HIV (Joint United Nations Programme on HIV/AIDS [UNAIDS], 2011). Even with successful roll out of prevention of mother to child transmission programs in Tanzania, there were nearly 29,000 perinatal infections in 2009. As of December 2010, only 18% of these were enrolled in HIV treatment programs (UNAIDS, 2011; World Health Organization [WHO], 2011). Without antiretroviral therapy (ART), one-half of HIV-infected children will die before their second birthday. Since clinical research to determine efficacy and safety of new medical treatments is conducted almost exclusively in adults, provision of new medical treatments to [End Page 75] children always lags behind advances for adults. Furthermore, children are often underrepresented in programs for antiretroviral drugs for the following reasons: 1) they are not prioritized for care; 2) facilities lack infant diagnostic capabilities; 3) there is limited pediatric HIV training and consequently, a shortage of pediatric providers; and 4) pediatric antiretroviral formulations are frequently unavailable. How important is the ability to freely control eye movements for reading comprehension? And how does the parser make use of this freedom? We investigated these questions using coregistration of eye movements and event‐related brain potentials (ERPs) while participants read either freely or in a computer‐controlled word‐by‐word format (also known as RSVP). Word‐by‐word presentation and natural reading both elicited qualitatively similar ERP effects in response to syntactic and semantic violations (N400 and P600 effects). Comprehension was better in free reading but (...) only in trials in which the eyes regressed to previous material upon encountering the anomaly. A more fine‐grained ERP analysis revealed that these regressions were strongly associated with the well‐known P600 effect. In trials without regressions, we instead found sustained centro‐parietal negativities starting at around 320 ms post‐onset; however, these negativities were only found when the violation occurred in sentence‐final position. Taken together, these results suggest that the sentence processing system engages in strategic choices: In response to words that don't match built‐up expectations, it can either explore alternative interpretations (reflected by regressions, P600 effects, and good comprehension) or pursue a “good‐enough” processing strategy that tolerates a deficient interpretation (reflected by progressive saccades, sustained negativities, and relatively poor comprehension). People can support abstract reasoning by using mental models with spatial simulations. Such models are employed when people represent elements in terms of ordered dimensions (e.g. who is oldest, Tom, Dick, or Harry). We test and find that the process of forming and using such mental models can influence the liking of its elements (e.g. Tom, Dick, or Harry). The presumed internal structure of such models (linear-transitive array of elements), generates variations in processing ease (fluency) when using the model in (...) working memory (see the Symbolic Distance Effect, SDE). Specifically, processing of pairs where elements have larger distances along the order should be easier compared to pairs with smaller distances. Elements from easier pairs should be liked more than elements from difficult pairs (fluency being hedonically positive). Experiment 1 shows that unfamiliar ideographs are liked more when at wider distances and therefore easier to process. Experiment 2 replicates this effect with non-words. Experiment 3 rules out a non-spatial explanation of the effect while Experiments 4 offers a high-powered replication. Experiment 5 shows that the spatial effect spontaneously emerges after learning, even without a task that explicitly focuses on fluency. Experiment 6 employed a shorter array, but yielded no significant results. (shrink)

Video gaming, specifically action video gaming, seems to improve a range of cognitive functions. The basis for these improvements may be attentional control in conjunction with reward-related learning to amplify the execution of goal-relevant actions while suppressing goal-irrelevant actions. Given that EEG alpha power reflects inhibitory processing, a core component of attentional control, it might represent the electrophysiological substrate of cognitive improvement in video gaming. The aim of this study was to test whether non-video gamers, non-action video gamers and action (...) video gamers exhibit differences in EEG alpha power, and whether this might account for differences in visual information processing as operationalized by the theory of visual attention. Forty male volunteers performed a visual short-term memory paradigm where they memorized shape stimuli depicted on circular stimulus displays at six different exposure durations while their EEGs were recorded. Accuracy data was analyzed using TVA-algorithms. There was a positive correlation between the extent of post-stimulus EEG alpha power attenuation and speed of information processing across all participants. Moreover, both EEG alpha power attenuation and speed of information processing were modulated by an interaction between group affiliation and time on task, indicating that video gamers showed larger EEG alpha power attenuations and faster information processing over time than NVGs – with AVGs displaying the largest increase. An additional regression analysis affirmed this observation. From this we concluded that EEG alpha power might be a promising neural substrate for explaining cognitive improvement in video gaming. Since 1790, the term Naturwissenschaften occurs in the lecture lists of the University of Jena published in the Allgemeine Literatur‐Zeitung of Jena. Naturwissenschaften is used as a title for lectures previously listed under the headings of Philosophie or Naturgeschichte. The introduction of the concept of Naturwissenschaften is interesting for several reasons: Firstly, at that time it is not the usual label in this context, and one therefore has to ask whether it already implies the connotations that are associated with the (...) modern concept of Naturwissenschaften, i. e. science in the modern sense. Secondly, the lecture lists of the University of Jena give insight into the various changes in the established ordering scheme for academic lectures that lead to the introduction of the term Naturwissenschaften, thus contributing to a better understanding of both the history of this concept and of the development of the university faculties as we know them. A close look at the concept of Naturwissenschaften as it was used around 1800 may, thirdly, help to understand the use made of this concept in philosophy, especially in the philosophy of nature of this period. Paul Ziche entwickelt in seinem Beitrag „Philosophische Psychologie jenseits von Psychologismus, Phänomenologie und deskriptiver Psychologie: Oswald Külpes experimentelle Untersuchung philosophischer Probleme“ Külpes Konzeption einer philosophischen Psychologie, die einerseits den philosophischen Ansprüchen seiner Zeit gerecht zu werden beansprucht, andererseits aber an der Leistungsfähigkeit experimentell psychologischer Arbeit festhält, dabei aber den Spagat versucht, einem reduktiven Psychologismus zu entgehen. Dabei konzentriert sich der Beitrag auf die Experimente Külpes hinsichtlich philosophischer Begriffe und Probleme einerseits und der Ermittlung der Külpischen Strategien zu einem umfassenden (...) philosophischen Projekt unter dem Titel „Realisierung“ andererseits. Somit scheint sich der Psychologismusstreit in der Person Külpes gleichsam aufzulösen, was Ziche zu dem Versuch veranlasst, die Situation der Zeit nicht als eine Kontroverse, sondern vielmehr als eine Interaktion zwischen den Lagern zu verstehen. Dabei kann Ziche in seiner Rekonstruktion der experimentellen Arbeit Külpes zeigen, dass dieser das Verhältnis von Psychologie und Philosophie im Ausgang seiner Experimente als ein wechselseitiges „Stützen“ versteht. Zudem gelinge es Külpe – so Ziche – die Kritiken, welche von beiden Lagern gegen seine Experimente ausgingen, positiv zu nutzen und als „komplexe Grundlagen aller Wissenschaften“ auszuweisen. In this paper we discuss Brandom's definition of necessity, which is part of the incompatibility sematnics he develops in his fifth John Locke Lecture. By comparing incompatibility semantics to standard Kripkean possible worlds semantics for modality, we motivate an alternative definition of necessity in Brandom's own terms. Our investigation of this alternative necessity will show that - contra to Brandom's own results - incompatibility semantics does not necessarily lead to the notion of necessity of the modal logic S5.

In an opinion piece penned at the Great War’s onset yet apparently unpublished until now, the historian of religion Hermann Gunkel outlined the opportunities he saw for the German people in anticipation of their triumph. He believed this war could consummate what the Napoleonic Wars and the Unification of Germany had not. Gunkel hoped for true German unity, more liberal domestic politics, and spiritual restoration.
